To serve as a link between patients, physicians, and David
S. Zocchi Brain Tumor Center, the care coordinator ensures that
the appropriate education, support and resources are provided. Which
includes:
- Consultation:
- Describe
the comprehensive spectrum of services available at David S.
Zocchi Brain Tumor Center for patients and families.
- Assessment:
- Perform
the detailed assessment to identify the individual needs for
patients and families, and provide the education, information,
and resource material.
- Follow-Up
Care:
- Conduct
follow-up to reduce stress and provide ongoing support.
- Assist
with the coordination and scheduling of tests and services
to ensure they are done in a timely and cost-effective manner.
- Communication:
- Communicate
with physicians, case managers, support services personnel,
families and others to ensure that patients receive necessary
services
